I actually don't think enough is being made of that letter. Releasing a fake medical letter (as that very likely is, for the reasons detailed in the article) would be a huge setback to his campaign. Why have there been no efforts to force Trump to substantiate that letter?

I don't think it's necessarily a fake, at least not a forgery in the normal sense of the term. I think it's entirely possible this doctor is just another Trump sycophant who wrote exactly what Trump wanted him to. Maybe even Trump told him exactly what to say and the doctor just put his name on it to make it official.

I'd like to think a doctor wouldn't do something like that but then I see all these anti-Clinton docs popping up in the media making a diagnosis of a patient they've never examined so it's really not all that hard to believe.

Yeah, iirc he's like 6' 1" and 215 lbs, so overweight but not too crazy. Still, if he's got chicken legs all his fat may be in his torso and he may look like that.

Though Trump's visit is not with the blessing of the state's governor:

Trump’s decision to tour the flood-ravaged areas was met with harsh words from the Democratic governor of Louisiana, John Bel Edwards.

Edwards also told Maddow that the President had called him early in this flooding crisis, declared a disaster as soon as Louisiana requested it, and deployed all of the resources requested by the state....
