The mods can track the IP address from which any comment was posted. They can also compare the email addresses each member used at registration. Beyond that it's a matter of judgement based on posting history and style.

And some do: recall our repeat offender Atheistoclast, who managed over 100 sockpuppets, some of which lasted months.

ISTAA is also another example - apparently his sockpuppetry goes back some time.
