Maybe Hollywood works like the mafia, you can't advance until they have enough dirt on you to hold against you as a safeguard.
In such an environment whistle-blowers are blacklisted. I'm sure in the old days "open your mouth and you'll never work in this industry again" was a very real threat. Well, except that opening your mouth was often a requirement.
