The mod itself is free and an alpha, yes.
You need either ArmA II Combined Operations (about 30$) or the free ArmAII version + the expansion called "Operation Arrowhead" (about 15$, but no HD graphics, I think.) to play it, though. Haven't properly tried ArmA II itself yet. Only the tutorial, in which I royally sucked. From what I read it's a rather good mil-sim, if you're into that sort of stuff.
It's also quite demanding on the hardware, especially the hard-drive. I can run it on my 5-year-old Toshiba Satellite p200, but with pretty much everything set to normal or low. Still looks nicer than you'd expect, though.
